Made To Dance Waiver & Release

Introduction

By booking or participating in any class, program, workshop, Social Experience, private lesson, event, or other activity offered by Made To Dance, Inc. ("Made To Dance"), I acknowledge and agree to the following:

1. Assumption of Risk

I understand that dance and other physical activities involve inherent risks, including slips, falls, collisions, strains, sprains, physical contact, and other injuries. Some activities may include partner work, floorwork, heels, turns, jumps, or other physically demanding movements.

I voluntarily participate and accept the ordinary and inherent risks associated with these activities.

2. Health & Personal Responsibility

I confirm that, to the best of my knowledge, I am physically able to participate safely.

I will inform my teacher or Made To Dance staff of any injury, medical condition, pregnancy, recent surgery, or physical limitation that may affect my participation. I understand that it is my responsibility to participate within my abilities, follow safety instructions, and stop or decline any activity I believe I cannot perform safely.

3. Partner Work & Physical Contact

I understand that some activities may involve partner dancing, physical contact, partner rotation, or physical guidance from teachers.

I may communicate my boundaries or decline a particular activity, and I agree to respect the safety, boundaries, and consent of other participants.

4. Release of Liability

To the fullest extent permitted by law, I assume the ordinary and inherent risks of participation and release and hold harmless Made To Dance, Inc., its directors, officers, employees, teachers, contractors, and representatives from claims arising from those inherent risks or from my own actions while participating in Made To Dance activities or using its facilities.

This waiver does not exclude or limit any liability that cannot lawfully be excluded under applicable Philippine law.

5. Emergency Authorization

In the event of an accident or medical emergency where I am unable to communicate a decision, I authorize Made To Dance to contact emergency services and take reasonable steps to obtain medical assistance on my behalf.

I accept responsibility for any resulting medical or transportation expenses, except where otherwise required by law.

6. Acknowledgment

I agree to follow Made To Dance's reasonable studio and safety policies and understand that I am responsible for my personal belongings while at the studio.

I confirm that I have read and understood this waiver and voluntarily agree to its terms.

7. Photo & Video

I understand that photography and video recording may take place during Made To Dance classes, Social Experiences, workshops, and events for community, documentation, and promotional purposes.

If I do not wish to be intentionally photographed, recorded, or featured in Made To Dance promotional content, I may inform the Front Desk or a Made To Dance staff member before the activity. Made To Dance will make reasonable efforts to respect my preference.

I understand that declining to appear in promotional photos or videos will not affect my ability to participate in Made To Dance activities.

I also agree to respect the privacy and preferences of other participants when taking my own photos or videos.

Minors

If the participant is below 18 years old, this agreement must be accepted by a parent or legal guardian on the participant's behalf.
